I/we acknowledge the receipt of and agree to the terms and conditions of my account(s) as stated in the "Understanding Your Accounts" disclosure and any amendments the Credit Union makes to these documents from time to time. Some accounts may require credit approval and your rates may vary based on credit history. SharePoint Credit Union is authorized to check my/our account, credit, identity and employment history and to obtain a consumer report from third parties, including credit reporting agencies to verify my/our eligibility for the accounts and services requested in connection with this application and for any update, renewal, or extension of credit or services to the extent permitted by law.

If VISA card(s) and/or MasterCard Debit card(s) is/are approved and issued, I/we agree to be bound by the cardholder disclosure and agreement included with the card(s). If approved for Reserve Line of Credit, I/we agree to be bound by the terms and conditions of the Reserve Line of Credit Account Agreement and Disclosure as provided to me/us upon signing or accessing the Reserve Line of Credit account and agree to repay any amounts I/we owe under the Reserve Line of Credit account in accordance with its terms. I/we understand that any of these terms may be changed by the Credit Union from time to time.

The Internal Revenue Service does not require your consent to any provisions of this document other than the certifications required to avoid backup withholding. Wisconsin Residents Only: Wisconsin law provides that no agreement, court order, or individual statement applying to marital property will affect a creditor's interests unless prior to the time credit is granted the creditor is furnished with a copy of the agreement, court order or statement, or has actual knowledge of the adverse provisions.

By entering your initials below, you certify that all information is accurate and complete, and both of you agree to be jointly and severally liable to repay any requested loan as required by the credit union. The credit union may verify this information from whichever sources it deems necessary (including credit reports) and may provide others with information regarding your credit history to the extent permitted by law. This application is, and shall remain, the property of the credit union. Your share balances and deposits with the credit union are subject to a lien by the credit union to secure any loan or other obligation owed to the credit union.
